Criteria for Evaluation · Professional/Career Achievement - - - - 60 points · Contribution to the development of the School/Community - - - - - - - 40 points 100 points Nominees will be screened/judged based on the description of the different categories: ¨ Education 1. The individual has given honor to SAC community and nation through his/her outstanding performance in education. 2. Has served the advancement of the profession through involvement in professional organizations or creation of professional development programs. 3. Manifest profound commitment to the development of the youth through exemplary competence, remarkable dedication to their work and effective educational leadership. 4. Have made a significant contribution to national development by raising the quality of outcomes of education through their effective, innovative and creative teaching strategies, materials and devices and their admirable efforts to bring the school closer to the community and consequently, the community to the school, thereby providing models of excellence in the teaching profession. ¨ Catholic Formation Ministry 1. The individual has given honor to SAC community and nation through his/her outstanding performance and service in the Christian Formation Ministry. 2. The individual has an outstanding record in the field of Christian Formation Ministry as a priest, nun, lay leaders and other religious related endeavor. 3. The individual has received awards and citation from various sectors in the province, country or international community as a recognition for his/her achievements and efforts in Christian Formation Ministry. 4. The individual has served SAC/Parish Diocese. ¨ Community Services 1. The individual provide a range of services and interventions to client and/or implement community intervention work may take place in community based organization. Residential services and outreach services. 2. The individual is involved in service delivery, either direct client work and/or health promotion and community developmental projects. 3. The individual has given honor to SAC community and nation through his/her outstanding service in the community. 4. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in community service.
¨ Business and Entrepreneurship 1. The individual has given honor to SAC community and nation through his/her outstanding performance in business and entrepreneurship. 2. The individual has an outstanding record in the field of business and entrepreneurship. 3. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in business and entrepreneurship. ¨ Auditing 1. The individual has given honor to SAC community and nation through his/her outstanding performance in Auditing. 2. The individual has an outstanding record in the field of Auditing. 3. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in Auditing. ¨ Engineering 1. The individual has given honor to SAC community and nation through his/her outstanding performance in the field of Engineering. 2. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in Engineering. 3. He/She has been active in professional practice and is an outstanding role model for current engineering students ¨ Science and Research 1. The individual has given honor to SAC community and nation through his/her outstanding performance in Science and Research. 2. ¨ Health Services
1. A graduate and/or licensed in any of the following professions: Medicine, Nursing, Physical Therapy, Med. Tech., Midwifery and other health related courses. 2. He/She has been active in professional practice. 3. He/She has given honor to St. Anthony’s College, the community and the nations through outstanding performance in health services. 4. He/She has received recognitions, citations or awards from his society or any associations. ¨ Information Technology 1. The individual has given honor to SAC community and nation through his/her outstanding service in information technology. 2. The individual has performed a variety of duties that range from installing applications to designing complex computer networks and information databases. 3. The individual has accomplished data management, networking, engineering computer hardware, database and software design, as well as the management and administration of entire systems. 4. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of hi/her achievements and efforts in Information Technology. ¨ Government Service 1. The individual has given honor to SAC community and nation through his/her outstanding service in government. 2. The individual has an outstanding record as a government employee. He/she has shown an effort to honestly rise from the ranks, achieve a position with exemplary reputation and service record. 3. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in government service. ¨ Culture and Arts 1. The individual has given honor to SAC community and nation through visual arts or theater and has shown extra ordinary talent in this field. 2. The individual was able to participate and earned an award in a Regional, National, Asian or Worldwide competition. 3. The individual has contributed to the development of literary, theater, visual arts, music or performing arts 4. The individual has received awards and citation from various sectors in the province, country or international community as recognition for his achievement and efforts for the development of literary, theater, visual arts, music and performing arts.
¨ Sports 1. The individual had given honor to SAC community and nation through sports and has shown extra ordinary talent in his field. 2. The individual was able to participate and earned an award in a Regional, National, Asian or Worldwide Competition. 3. The individual has contributed to the development of sports in the community. 4. The individual has received awards and citation from various sectors in the province, country or international community as recognition for his athletic achievements and efforts for sports development. ¨ Seamanship 1. The individual has given honor to SAC community and nation through his/her outstanding service in Seamanship 2. The individual has an outstanding record as a Seaman. He has shown an effort to honestly rise from the ranks, achieve a position with good reputation and service record. 3. The individual has received awards and citation from various sectors in the province, country or international community as a recognition of his/her achievements and efforts in Seamanship. ¨ Agriculture 1. The individual had given honor to SAC community and nation through agriculture and has shown extra ordinary talent in this field. 2. The individual was able to participate and earned an award in a Regional, National, Asian or Worldwide Competition. 3. The individual has contributed to the development of agriculture in the community. 4. The individual has received awards and citation from various sectors in the province, country or international community as recognition for his achievements and efforts for Agriculture. ¨ Journalism 1. The individual had given honor to SAC community and nation through journalism and has shown extra ordinary talent in this field. 2. The individual was able to participate and earned an award in a Regional, National, Asian or Worldwide Competition. 3. The individual has received awards and citation from various sectors in the province, country or international community as recognition for his achievements and efforts for journalism.
